Nurse Participation in No Harm Near Miss Error Reporting: A Quality Improvement Initiative
No Harm and Near Miss (NHNM) safety event rports play a crucial role in enhancing patient safety by identifying hidden risks within healthcare systems. By learning from these events, healthcare teams can make essential improvements to prevent future incidents. However, barriers such as insufficient training and feedback deter frontline nurses from filing NHNM reports. A targeted intervention using the PREACH method—Pausing, Reflecting, Engaging, Acting, Checking, and Helping—significantly boosted bedside nurses' willingness to report NHNM medication errors. Nurses who participated emphasized the importance of highlighting both the positive impact and the minimal time required for NHNM reporting, suggesting that these elements are key to the sustainability and success of this quality improvement practice.
Unacknowledged CPOE Improvement Project
The integration of Computerized Physician Order Entry (CPOE) systems alongside Electronic Medical Records (EMR) has become fundamental in hospitals, helping to safeguard patient safety by streamlining medication orders and care directives. However, even with CPOE systems, medication administration errors can still occur. This improvement project focuses on addressing these unacknowledged gaps, aiming to enhance the reliability of CPOE in reducing medication-related incidents and advancing patient safety in clinical settings.
